How to Change Surname in LTFRB

Changing the surname in LTFRB records is a simple procedure completed at the LTFRB ISMD. It is usually done following a change in civil status, be it because an operator wants to take on her spouse’s last name or use a hyphenated version of her last name and her husband’s last name after marriage. Other surname change requests also include reverting to the old last name following a divorce, annulment, or legal separation.  

Name changes such as this one doesn’t have much effect on the operation of PUVs. More often than not, it is a personal decision to do so in order to keep the LTFRB records as well as the business records updated to match the personal identification documents of the operator. While the change is nothing major, it is an equally important procedure as most transactions at the LTFRB require the submission of a valid government-issued identification document to complete. 

Qualifications 

Only the operators of public utility vehicles are required to go through the LTFRB for specific transactions like name change. That said, CPC holders who intend to seek authorization for change of name due to change of civil status can simply file the request at the LTFRB Office closest to them.

Requirements

To initiate the process of updating the name (surname) due to a change in civil status, applicants must prepare the following requirements:

  • Four (4) original copies of the Sworn Petition/Application Form for Change of Name due to Change of Civil Status from the LTFRB of via their website via the following links:
  • One (1) Original copy for presentation and one (1) Photocopy of any valid government-issued ID Card  
  • One (1) Original copy for presentation and one (1) Photocopy of Notarized Special Power of Attorney (For Authorized Representatives)
  • One (1) Original copy and one (1) Photocopy of Marriage Certificate issued by the Philippine Statistics Authority (PSA)
  • One (1) Original copy and one (1) Photocopy of Court Order / Judgment, if applicable 
  • One (1) Original copy and one (1) Photocopy of Official Receipt of payment for Change of Name due to Change of Civil Status  
  • Franchise Verification (to be provided by the LTFRB)

For Authorized Representatives (Individual Operators)

Personal appearance of the petitioner is required. However, if not possible for petitioner to be physically present, authorized representative is allowed upon presentation of: 

  • One (1) Original copy for presentation and one (1) Photocopy of Duly notarized Special Power of Attorney (SPA) 
  • One (1) Original copy for presentation and one (1) Photocopy of Valid Government-Issued ID of the applicant and authorized representative 

Procedures

Step 1. Visit the LTFRB Office

Begin by visiting the nearest LTFRB office and securing the Notice of Change of Name due to Change in Civil Status form and checklist of requirements at the LTFRB Public Assistance Complaint Desk (PACD) window.

Step 3. Fee Assessment and Payment

Wait for the assessment of fees to be completed and the payment order slip (POS) to be issued. Then, proceed to make payments at the cashier.

Step 4. Submission and Evaluation

Submit the request along with the official receipt of payment at the Information Systems Management Division (ISMD) Help Desk for further evaluation by the Technical Division.

Step 5. Wait for Evaluation

Wait for the IT officer to complete the procedure and update the information in the Franchise Processing System. 

Step 6. Claim Updated Franchise Verification

Upon approval, visit the LTFRB office and claim the updated Franchise Verification reflecting the changed name from the ISMD.

Fees

The request to update names due to change of civil status is not free. When you file a petition requesting for one, you need to be ready to pay the fees which include the following:

  • Filing Fee: PHP 250.00
  • Legal Research Fee: PHP 10.00
  • Franchise Verification Fee: PHP 40.00

Processing Time

The processing time is estimated to be at least eight (8) hours, though it may vary depending on the completeness of the requirements submitted. Having incomplete requirements may also affect the time needed to complete the procedure.
